Overjet vs Pearl:

Many clinicians use both Overjet and Pearl. But when they test both side by side, they choose Overjet for its clinical accuracy and depth. Let’s take a look at why:

Overjet is the pioneer of segmentation and quantification in dental AI. Until recently, it was the only FDA-cleared tool capable of segmenting, outlining, and quantifying oral disease, whereas Pearl and other AI systems were only cleared to place bounding boxes around areas of suspected oral disease.

While Pearl recently received clearance for segmenting caries in patients aged 19 and older, we are flattered to hear Overjet was listed as the predicate device in their clearance. Moreover, Overjet remains the only AI cleared for detecting pediatric caries, anatomical structures, and periapical radiolucencies (PARLs).

Additionally, only Overjet's AI can sync periodontal charts from PMS software to identify patients with potential periodontal disease and those due for charting.”

Here are some examples from a doctor that tested the two software's side by side on the same images.

Overjet correctly measures bone levels from the identified CEJ,, while Pearl overestimates bone levels on the mesial of #7 and the distal of #8 due to an inaccurate AI model. Additionally, Pearl identifies a false positive carious lesion on the distal of #8. Furthermore, Pearl uses millimeter (mm) measurements on periapical (PA) images, which leads to a larger margin of error due to angulation. In contrast, Overjet measures in percentages, which aligns with the agreed-upon standard set by the American Academy of Periodontology guidelines.

For tooth #3, Pearl fails to identify both the distal area with recurrent decay and the short prosthetic margin on the mesial. Additionally, it poorly measures bone levels on #5, 6, 28. In fact on teeth 5 and 6 Pearl is arbitrarily measuring to end of the x-ray sensor vs looking for bone. Pearl’s visual output is also cluttered with bounding boxes intended to highlight restorations. In contrast, Overjet’s analysis of the same image hides the bone level markings on tooth #3 to allow for a clearer clinical appreciation of the recurrent decay on the distal marginal interface (marked in red) and the short margin on the mesial of #3 (highlighted with a yellow box). Moreover, Overjet provides significantly improved clinical accuracy in bone level assessments throughout both the upper and lower quadrants.

Overjet’s enamel tracing is far more precise compared to Pearl’s. Additionally, Pearl produces a false positive for decay on the mesial of #7 and inaccurately measures bone levels. Overjet correctly identifies recurrent decay on the mesial of #9, which Pearl’s AI decay algorithm misses, while also offering a clearer and more minimalistic clinical presentation.

Overjet delivers significantly more accurate bone level measurements, whereas Pearl exhibits both overestimation and underestimation within the same image. In this case, the patient presents with severe bone loss, yet Pearl’s measurements inconsistently indicate both bone loss and normal levels on either side of the same tooth, despite clear horizontal bone loss. Additionally, Pearl incorrectly identifies endodontic involvement on tooth #26, indicating a periapical radiolucency (PARL) with a blue box. Measurements for tooth #24 have been turned off to better illustrate the horizontal bone loss.
